Tips for running the diagnostic
💡 Allocate time during a meeting for your team to complete the survey.
This consistently works better than sending the link in advance.

How to set up your team

Choose when your team will complete the survey (this will take 5 - 10 minutes).
Create a code by using the date and 3 letters from your school/organisation name.
For example 25 March Dr Selena Fisk = 2503DSF
Fill out the form to let us know your chosen date and code and who we should email the completed report to.
Please note that your summary report must be paid for prior to it being sent to you.
If you haven’t ordered your report through the online shop, and you wish to be invoiced, simply check the box on the form to let us know.
Once your team has completed the diagnostic, your summary report will be emailed to you within 7 days.

All responses in the summary report are anonymous
Only Shona and Selena can access names/emails, and only to remove duplicates/track down responses where no code was entered
If you choose not to provide details
Your responses will still be included in the summary report (as long as you entered your school code)
You won’t receive your individual PDF report via email
